Savings Rebate Program

Disclosures: 1. THIS PROGRAM IS NOT INSURANCE. 2. The Savings Program only applies to paid-in-full, in-network contracted doctor office visits with a total billed amount of $500.00 or less. Procedures relating to Cosmetic, Lasik, Global Maternity, Infertility, Gastric Bypass, weight reduction, Ancillary (labs and imaging) procedures, Social Workers/Services and or out-patient and in-patient Urgent Care/hospital facility procedures are excluded from the savings rebate program. 3. All forms not completed in their entirety and returned within 90-days from the date of service we will not be processed. 4. The program provides discounts at certain health care providers for medical services. 5. The program does NOT make payments directly to the providers of medical services. 6. Program members are required to pay for all health care services but may receive a discount from those healthcare providers who have contracted with the discount medical plan organization. 7. Note to Utah residents: This contract is not protected by the Utah Life and Health Guaranty Association. The program and its administrators have no liability for providing or guaranteeing service by providers or the quality of service rendered by providers.
